Methods: We conducted a prospective, multicenter, randomized, double-blind, placebo-controlled, parallel-group study that compared the effectiveness of fluconazole and diflucan in patients with a first-time vaginal candidiasis. Patients were treated with fluconazole, oral fluconazole (400 mg, 100 mg twice daily), and diflucan (50 mg, 150 mg, or 200 mg) for one week, then fluconazole and diflucan were combined for the second week and diflucan was added at a dose of 200 mg or 50 mg. The primary outcome was the effectiveness of fluconazole and diflucan versus placebo in the first-time vaginal candidiasis. Secondary outcomes were time to first candidiasis recurrence, time to yeast recurrence, number of candidiasis-related infections and candidal shedding, and clinical cure. A total of 1,716 patients were enrolled and 1,058 (94.8%) received fluconazole and diflucan, respectively. The median age was 51 years, 47.6% were female, and 55.2% were white. The study population consisted of 735 men (14.4%) and 792 women (20.2%). Fluconazole was most effective for fungal and candida infections, with a median time to first recurrence of 2.4 months (95% CI: 1.1-3.8) and 7.3 months (95% CI: 3.1-12.5), respectively. The study population consisted of 716 men (29.8%) and 892 women (20.2%). Fluconazole and diflucan were equally effective for vaginal candidiasis in the first-time vaginal candidiasis.
